Meal Plan Change Form

All residential students automatically receive 19 meals per week. A 14 meal per week plan is also available. Meal plans may be changed to 14 meals per week or changed back to 19 meals per week until the end of the first week of classes of each semester. Meal plans will remain until a change form is received in the Office of Student Life.

If you would like your meal plan changed at this time, please complete the following form.

1. I want to change my meal plan from 19 to 14 meals per week (a reduction of $40.00 per semester).
